An online survey is underway to assess the health needs of Sangamon County residents. The county health department is teaming up with HSHS St. John’s Hospital and Memorial Medical Center to ask residents about their health concerns and the availability of treatment for them.
The survey… which is required under terms of the Affordable Care Act… will be online until February 12th . You can take the Community Health Needs Assessment at go.uis.edu/chna.
